Use the current district application form to build your checklist. Organisation evidence, programme details, signatures, co-organiser details and fee-waiver evidence may differ between districts and applications. This worksheet records the requirements you have confirmed; it does not replace the official form or determine eligibility. Keep document files private and record their names or locations here rather than personal identification details.
How to use it
- Open the current official form for the chosen facility and room.
- Enter only the requirements relevant to that application and give each an owner.
- Record the submission reference and track the written result separately.

Requirements come from the actual form
A previous event’s checklist is a starting point, not evidence that the same documents still apply. Confirm which organisation is applying and whether joint organisers or a fee-waiver request need supporting material. Record an unanswered question instead of treating a missing document as unnecessary.
Submission and booking confirmation are separate
Save the submission record and the version of the programme submitted. When a result arrives, record the approved room, date, session and any conditions. Update the run sheet only after checking those details; changes may need further approval.
